Please notify the Rochester Christian School office of any phone number/contact changes. It is essential that home and work numbers for parents and emergency contacts are correct.
Immunization Requirements
Students entering preschool and grades PreK-12 must be fully immunized according to NYS Public Health Law. Please check with your healthcare provider to ensure your child meets age/grade requirements. Students not in compliance may NOT attend school until the requirement is met.
Medication in Schools
The NYS Education Department requires new medication orders every academic year. A provider’s order and parent permissions are required for all prescription and non-prescription medication if taken during the school day. If a parent requests OTC medications the physician must fill out the OTC form provided by school/parents or send their own orders according to parents needs. Tylenol and Ibuprofen orders must have doses and will not go by package labels.
MEDICATIONS WITHOUT A PHYSICIAN SIGNATURE WILL NOT BE ACCEPTED.
HIPAA
HIPAA forms are used to allow the school nurse to obtain school health documents from providers. Documents such as physicals, immunizations, and medication orders can easily be faxed back and forth from the provider with your permission. This is not a requirement, however it is helpful in order to retrieve information without calling parents back and forth for what is needed. This form is only used by the health office.
Medication in School Policy
Prescription medication must be in the original pharmacy container, labeled with the student’s name, medication, and dose. Non-prescription medication must also be in the original container with the student’s name on the bottle/box. All medication MUST be delivered to the health office by the parent/guardian. Students are NOT permitted to carry any medications to school. All medications will be stored in the health office in a locked cabinet.
State Screening Mandates
NYS mandates Vision, Hearing and Scoliosis screenings – Parents/Guardians will be notified of any significant findings. Required Physicals are for grades: K-1-3-5-7-9-11
